Hello, my name is Teresa Ransom and I am the Care Coordination Program Director at Cornerstone Home Care. I was born and raised in Juneau, and my mother and I both graduated from JDHS. I am about as local as they get. I am always asked why I stay in Juneau because my brothers and sisters live all over the country: Las Vegas, Hawaii and California. I always answer the same way: “I stay for my family.” I have cousins, aunts, uncles and grandparents who live all over Juneau. I love running into them when I am out running errands. Additionally, staying close to home guarantees that I always have somewhere to go on holidays. My family is very important to me. My philosophy is to make sure that all of my clients get the care that I want my own grandmother to get if she should ever need them. 
I first started at Cornerstone as an administrative assistant in 1999; I was hired to answer phones and type labels. I then moved up to be the Assistant to the clinical director for our skilled nursing program. It was my job to track orders, assessments, plans of care and prior authorizations for insurance companies - among other things. It was my experience as an assistant that provided excellent skills and a suitable background for my current position.
 
As the Care Coordination Program Director, I oversee the care coordination services that CHC is currently providing in Juneau, Haines, Angoon, Kake, and Petersburg. I currently have care coordinators in the Juneau and Haines offices that report directly to me. All Care Coordinators are trained to work well with other provider agencies. We all work together to make sure our clients get the best care possible. Care Coordinators are, by nature, creative problem solvers.
