Independent Living Services

The goal of the program is to help people remain safely independent in their own homes and communities. All services and programs are supervised by a Registered Nurse. Services include:

Personal Care
 

• Agency Based Personal Care is available to anyone who requires assistance with activities of daily living (bathing, grooming, meal preparation, etc.) Agency based trained staff will assist with a variety of tasks identified during your free assessment.
   
  Consumer Directed Personal Care is available to Medicaid recipients who need assistance with activities of daily living and have a suitable person who is willing to be paid for providing this hands-on care. Call us to see if you qualify for a free CDPCA assessment.

Care Coordination

 
The goal of the Care Coordinator is to assist the client in identifying and obtaining the services necessary to allow the individual to remain safely independent in the least restrictive environment of their choice.
